Community Connectors are trained Social Prescribing Link workers lined up with local GP practices in Surbiton and Chessington, working with Staywell.
They can help you to make positive changes to your life by connecting you to volunteers, activities and voluntary/community groups, helping you to:

feel more involved in your community

meet new people

make some changes to improve your health and wellbeing

deal with things that can affect your health such as debt, caring responsibilities or loneliness
They encourage you to help yourself. They support and work with you to create and follow a personal action plan. They listen to you and focus on what matters to you. They will also help you to access services and/or resources wherever possible, in order to address any non- medical needs you may have. They are experienced in working with people with Dementia.
